This sheet gives us information about you and your pet. Most importantly used when you first join our clinic, but can also be handy when you get a new pet. In case your animal ever needs to stay with us overnight, this form needs to be filed with us. Only one form per client. It reminds you that we are not a 24 hour facility and that there are hours when your animal will be unattended. If you would not prefer this, there are emergency hospitals to which we can refer you for after hours.

| Surgical Consent Form |
This form gives us your permission to sedate your animal or perform surgery. It also lists what procedure to be performed as well as some other common procedures which are easier done under sedation or anesthesia. This form will need to be filled out before every procedure.

Bringing this form with you will save you time as you try to leave on your travels. This sheet details if you are bringing your own food, how long your pet is staying, and if any procedures or vaccines should be given. We will try to examine your animal when you come in for boarding and go over anything your pet needs while boarding.

We understand that treating skin issues can be very complicated, time consuming and expensive. By filling out this questionnaire, we can save you time and lots of questions. Having this extensive history will also help us from trying things that have already been done. Please take the time to fill this out before coming in with your pet with skin/fur problems!
